Community Health Need Assessment

Madelia, Minn. – Madelia Health is conducting an online survey to help assess the health needs of the communities we serve. It is an opportunity for community members to give feedback to us by identifying and analyzing the health needs of the people in the area. The survey results provide us with a way to plan and prioritize for the future.

Madelia Health is a tax-exempt organization, so we are required to conduct a Community Health Needs Assessment (CHNA) every three years. But, in addition to the survey being a requirement, it is an opportunity to get feedback on the health needs of the community and how Madelia Health is doing as an organization.

Please take ten minutes to complete this online survey for Madelia Health. Your input is confidential. Madelia Health is working with Eide Bailly as a consultant, and Eide Bailly is the only one who will see the individual responses. This survey is done only once every three years, so it is essential that community members respond. The results of the survey will be published and posted on the Madelia Health website early in 2022 at: www.madeliahealth.org.
