What is Transitional Care?

Transitional Care at Madelia Health, also known as Swing Bed, provides short term rehabilitation services for patients with skilled needs. Patients most appropriate for transitional care are those transitioning from an acute hospital setting to a home or a nursing home setting.

Skilled Services Offered:

  • Daily Occupational therapy to meet self-care goals

  • Daily Physical therapy to improve strength and mobility

  • Specialized wound care

  • IV Therapy

  • 24-hour nursing staff and emergency department

  • In-house pharmacy

  • Discharge planning

These services are individualized for each of our patients based on needs and personal goals.

Services are covered by Medicare and private insurance plans. Coverage is the same from facility to facility.  (Please note that each insurance plan is different.)

Swing Bed Program - South Green Room

Comfort Rooms Include:

Each patient will have a private room and bathroom, Smart-TVs, WiFi, and freshly prepared meals. They are welcome to bring their own clothing to maximize comfort.
