Description:

The Case Manager/Utilization Review RN will work to facilitate patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress. The Case Manager/Utilization Review RN’s role is to improve clinical outcomes, increase patient satisfaction, and promote cost-effectiveness. Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients, their family, and community resources ensuring that all services prescribed in the individualized care plan are implemented. Compile and review medical information of individual patients and the limited insurance coverage available to them. Balance the quality of patient care with cost efficiency, ultimately maximizing the continuity of care a patient receives. Consider the appropriateness or necessity of services before authorizing them or preparing cases for provider review. Manage insurance denials and appeals or work with patients to help them understand the limitations of their benefits.

Tasks & Responsibilities:

  • Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
  • Communicate patient needs to a variety of care team members and follow up accordingly
  • Manage discharge plans during stay and upon completion of treatment

  • Education: minimum Bachelor’s degree in Nursing from an accredited school of nursing

Madelia Health Home Care is seeking an experienced RN to join our team. This RN will manage a client case load, make visits to client homes, assess, plan and implement client care and perform staff supervision. Madelia Health is looking for trained Emergency Medical Technicians or Emergency Medical Responders to be part of our team. In the EMT role, you will provide basic life support transport care to patients during inter-facility transfers and respond to 911 calls as necessary. This would be an on-call volunteer position with the possibility of picking up paid shifts during weekdays.
