In-home Support

Madelia Health Home Care provides in-home support to persons recovering from a major illness or surgery and those living with chronic health conditions. Our clients receive customized care based on their individual needs to allow them to continue living in their own home. Services range from professional nursing care and physician-ordered therapies to personal care and light housekeeping. The home health team works to promote, maintain or restore the client’s health. Patient and family education along with coordination of medical care are essential elements of the success of a client remaining in the home.

Our mission is to provide personalized, compassionate care to improve the quality of life and to retain our client’s independence while allowing them to remain in the safety and security of their own home.

Our Care Team

  • Physician: Clients are encouraged to keep their own primary care physician. We maintain relationships with various area healthcare providers to ensure continuity of care.

  • Nurses: RNs and LPNs on staff assess client conditions, manage medications, perform wound care, and administer select IV medications. RNs formulate, initiate and supervise the client’s plan of care. Nurses provide education to clients and loved ones regarding their recovery, chronic conditions, medications and safety at home.

  • Certified Nursing Assistants: Our CNAs are certified to provide care by the State of Minnesota. These professionals can assist with personal care, bathing, homemaking, meal prep and errands.

  • Physical Therapy: Our team works closely with our therapists in Madelia and Lake Crystal as well as with St. James Area Rehab to provide in-home therapy. Physical therapy (PT) can help increase mobility, strength and endurance to assist clients to regain function.

  • Occupational Therapy: This therapy service assesses and aids clients to adapt their environment to promote independence and safety.

Available 7 days per week

Home care is available seven days per week and does provide 24 hour “on call” services to their clients. Duties performed by home care staff include:

  • Physical assessments

  • Medication Management

  • Monitor vital signs

  • IV therapy
  • Wound care

  • Post-surgical care

  • Diabetic education and management

  • Physical & Occupational Therapy
  • Provide emotional and psychosocial support
  • Assistance to manage your healthcare issues

  • Personal care and bathing assistance

  • Assistance with home exercise programs

  • Homemaking

  • Support to make informed decisions

  • Education regarding medications, illness & diseases

  • Connection to other community resources

Payment

Payment for home care services varies depending on client condition and qualifications. Payment for services may be provided by Medicare, Medicaid, Waivered Services with the surrounding counties, Private Insurance, Long Term Care Insurance, or private payment. Madelia Health Home Care proudly provides services to our nations heroes while working in collaboration with the Veterans Administration.  To inquire regarding home care services, please contact the Home Care Department at Madelia Health or discuss services with your physician or nursing staff.
